Washington based upon counsel's failure to raise a structural error must " in addition to demonstrating deficient performance " show that he was prejudiced by counsel's ineffectiveness, or whether prejudice is presumed because the harm from structural errors is "necessarily unquantifiable and indeterminate," Sullivan v. Louisiana.
